Background
Tanzania has achieved Millennium Development Goal 4 for child survival but more needs to be done, under-five child mortality continued to decline over the last 25 years. Infant mortality has decreased from 92 deaths per 1,000 live births in 1991/1992 to 43 deaths per 1,000 live birth in 2015/2016, thus, achieving the MDG 4 targets. Despite of these achievements, about 270 children under-five die every day mainly from preventable and treatable conditions, such as malaria, pneumonia and diarrhea. Between the years 2000 and 2010, under five mortality decreased by 43%, whereas the rate of reduction of neonatal mortality during the same period was 21%. Neonatal mortality in 2013 was at 21 per 1000 live births and contributes to 39% of under-five deaths, with nearly 40,000 new-born deaths every year.

Tanzania failed to achieve the Millennium Development Goal 5 of improved maternal survival: The reduction of maternal mortality has been slow. In 2015, the maternal mortality rate slightly increased from 432 in 2013 to 455 in 2016. Neonatal deaths are inextricably linked to the conditions of the mother, services utilization and the quality of care received during pregnancy, delivery and postpartum. The slow decline of both maternal and newborn mortality makes a compelling case for strategic prioritization of maternal and newborn care, especially around the time of delivery.

Even though antenatal care (ANC) coverage is high (98%) for the first visit, only 1 in 4 women had their first ANC visit in the first trimester as recommended and only 51% of women make the recommended four visits. Sixty-three percent (63%) of the deliveries occur in the health facilities and 37% of births occur at home. Women with no education, those living in rural areas and those in the poorest households are the most likely to deliver at home. However, there is some improvement as only 50% of deliveries were reported to take place at health facility in 2004-2005. Overall 64% of births are assisted by skilled provider. Only 34% of women receive postnatal care within 48 hours. There is high fertility rate (5.2%) as well as high rate of adolescent pregnancy, with 27% of girls aged 15-19 beginning childbearing. This strongly indicate the importance of family planning and adolescent health services as a prerequisite for reducing maternal deaths.

UNICEF continues to work closely with Ministry of Health Community Development Gender Elderly and Children (MOHCDGEC) and Local Government Authorities (LGAs) to support delivery of high quality health services. These services include reproductive and maternal health, immunization, child health, prevention of mother to child transmission of HIV, early childhood development services. However, having quality services available is not enough. There is a need to ensure that the services are fully utilized by the target population. Utilization of the services is currently not at optimal and desired level. Antenatal care (ANC) coverage is high (98%) for first visit, despite high and quality antenatal services only 1 in 4 women had their first ANC visit in the first trimester as recommended and only 51% of women make the recommended four visits. This is majorly attributed to late booking for antenatal care. Sixty-three percent (63%) of the deliveries occur in the health facilities and 37% of births occur at home. Wo
men with no education, those living in rural areas and those in the poorest households are the most likely to deliver at home. However, there is some improvement as only 50% of deliveries were reported to take place at health facility in 2004-2005. Overall 64% of births are assisted by skilled provider. Women who have attended 4 ANC visits, those living in urban areas and the wealthiest households and those with secondary or higher education are most likely to receive delivery assistance by skilled provider. Additionally, only 34% of women receive postnatal care within 48 hours. According to the Health Sector Strategic Plan July 2015-June 2020 high community engagement is required. However, relatively few communities are reached with comprehensive set of interventions.

Tanzania is currently developing a formal cadre of Community Health Workers (CHWs) through the Community Based Health Program (CBHP). The CHWs will be responsible to link health facilities to community; and support community led health promotion activities. The CBHP envisions communities with improved health that contribute to socio-economic development. Mission of program is to empower communities and build capacity through sustainable and integrated CBHP to respond to local health needs.

Justification
Successful development of functional and effective CHWs depends on collaborative efforts of stakeholders including UNICEF. The process demands adequate number of human resources within UNICEF. However, UNICEF for this moment has no plan to hire core staff to support community health activities.

Specific tasks
The intern will work under directive and guidance of the chief of health to complement execution of the following tasks:
i. Contribute to UNICEF’s processes of building capacity of the partners implementing CBHP.
ii. Contribute in monitoring of UNICEF’s partners implementing community health.
iii. Contribute in documentation reporting and sharing information on community health activities.

Expected background and Experience
The intern is expected to have the following background and experience:
• Bachelor’s Degree in the public health, social/behavioral sciences, (Sociology, Anthropology, Psychology, Health Promotion, community Development) with emphasis on strategic communication planning for behavior development, social mobilization, participatory communication, and research.
• Be fluent in English.
• Have excellent academic performance as demonstrated by recent university or institutions records.
• Additional consideration will be given for any past relevant experience.

 Tanzania’s public sector workers are paid better than their counterparts in the private sector, according to Employees and Earning Survey



Tanzania’s public sector workers are paid better than their counterparts in the private sector, according to Employees and Earning Survey, 2015.The survey, published recently by the National Bureau of Statistics (NBS), shows that the majority of public sector employees are paid more than Sh300,000 while most of those in the private sector earn less than Sh300,000.Tanzania had 2 million employees by 2015, of whom 1.28 million (63 per cent) were from the private sector, while 0.775 million were from the public sector.The survey indicates that seven out of ten formal employees in Tanzania, both public and private sector, earn monthly wage rate ranging from Sh100,000 and Sh500,000.

Half of private sector employees earn monthly salary ranging from Sh100,000 to Sh500,000, while the public sector, the majority (27 per cent) earn monthly salaries ranging from Sh300,001 to Sh900,000.

While two out of ten (1.7 per cent) public employees earn monthly salary ranging from Sh900,001 to Sh1.5 million, earnings for their counterparts in the private sector also stand at two out of ten (1.5 per cent).

However, the findings further reveal that the proportion of employees earning monthly wages above Sh1.5 million was slightly larger in private sector (2.1 per cent) than in public sector (1.9 per cent). On gender perspective, the majority of employees earning up to Sh100,000 per month were female at 8 per cent lower than 5.7 per cent male while male also dominated the highest percentage of those earning between Sh100,001 and Sh300,000 by 36.2 per cent higher than 31.3 per cent female. Employees earning monthly wage of between Sh300,001 and Sh500,000 were the same between male and female as both accounted for 24.7 per cent.


Financial and insurance professions are the highest paid jobs in Tanzania, after the employment and earning survey 2015 showed that they earn ten times of Sh150,000 minimum wage.

The survey shows further that three out of ten employees in insurance and financial sectors earn the whooping monthly salary of Sh1.5 million and above.

The report shows that five out of ten employees in the same sectors earn monthly income of Sh500,001 to Sh1.5 million.

The second highly paid jobs are in mining and quarrying as the survey shows six out of ten employees are said to earn monthly income of Sh500,001 to Sh1.5 million. Six out of hundred (15 per cent) earn monthly salary of Sh1.5 million or more.

Teachers and school administrators were listed as the third highest paid as the survey shows that five out of ten people earn Sh500,000 and above. However, only five per cent of the employees in the sector earn Sh1.5 million and above.

The lowest paid are those working in both administrative and support services and agriculture, fishing and livestock activities as eight out of ten are earning monthly salary ranging from Sh100,000 and Sh300,000.

Generally, the survey shows that public sector employees are paid handsomely than those working in the private sector.

Responsibilities of data collectors
To participate in daily clinical rounds in the surgical and obstetric wards and work with facility clinician to identify post-op patients with targeted complications for the project

• To ensure that proper consent has been obtained from all study subjects as required by the protocol .

• To teach Health facility clinicians and data collectors appropriate data collection techniques and oversee their collection

Location Of the activity:
This evaluation will be conducted in 10 intervention Health facilities in Kagera and Mara regions (5 in each region) and 10 control sites in another selected region. One data collector will be stationed in each of the 20 sites during the entire data collection period

Activity Dates
Baseline assessment is expected to last from 15 January to 15 March 2018, but this period may be extended up to six months since the data collectors will have to train the facility-based providers to continue data collection beyond the baseline assessment, hence the need to stay longer in these facilities

Required qualifications
The data collectors will be Medical Doctors who have graduated from a medical school and have completed internship in a recognized institution. They should have at least temporary registration with the Medical Council of Tanganyika and a certificate of internship.
