Form Four Index Number<br \/>\n2. CRDB Bank Card<br \/>\n3. Registration Number<br \/>\n4. Registration Document (for First Year Students)<br \/>\n5. ID Card for Second (Year Students)<br \/>\nService Centres for this important exercise will include all cafeterias. The Centres will be open from 8:00 am to 5:00 pm from 5th to 12th December, 2012.
